How can I request my medical records?
In order to get a copy of your medical records, you must first complete a HIPPA-compliant medical record release form and submit it to our office. After this is completed, then we can send your medical records to you via fox or email. This process ensures that your medical records are kept safe, secure, and confidential.

Do you have a cancellation policy for psychiatric services?
At Iremia Psychiatric Care in Tampa, FL, we understand that life can be unpredictable. However, if you must cancel or reschedule your appointment, we ask for a minimum of 24 hours’ notice. This policy allows us to offer the time to another patient who may need care.
If you miss your appointment or don’t reschedule it or cancel it in time, you’ll be charged a fee of $25. Please be aware that insurance will not pay for this fee. If you miss appointments repeatedly, your treatment may be disrupted and you could be at a higher risk of treatment failure.
For these reasons, we ask that you make every effort to come to your appointments.
Do you fill out paperwork and/or complete letters for mental health disorders? And do you charge a fee?
Yes, we do fill out necessary paperwork for mental health conditions. This includes disability paperwork, return-to-work forms, and letters for specific individuals or organizations such as FMLA, social security, or schools.
Please note that there’s a fee for this service. Requests for paperwork will be reviewed by Grecia to make sure they’re appropriate. Disability paperwork will be completed at the discretion of Grecia and after you’ve attended at least three appointments.
Please allow a minimum of 3-5 business days for paperwork to be completed.
